Before Your Visit

Before your first  visit, there are some forms you will need to fill out and bring with you. You can download these forms here. Forms are in .pdf format, so you will need Adobe reader to view them. If you don't have Adobe reader, you can get it HERE (link will open in a new window).

Before your ablation or phlebectomy procedure, you must fill out a consent form, which you will find below. You will also find pre- and post-op instructions for your procedure.

If you are undergoing the sclerotherapy procedure, you will need to download the forms below.

For all procedures, you will need compression stockings. Instructions on how to wear your stockings (including a link to a YouTube video demonstration) are below.
